It is mandatory to fill out this form once you have been accepted. These cards are accessible (in a physical form) by all of the players in the Manor. All information that you enter becomes public knowledge to every character in the game. Please do not omit anything important, and be truthful, even if it is something your character doesn't want others to know about; the characters themselves aren't filling out these cards- they are provided by those running the show, and how they received the information will be unknown.
